No difference. This sandal gives me the proper support without the orthotic. The first set of my regular shoe size I felt to be a little loose so I purchased the next size down and found to be way too small. The sole pulled apart from the shoe. So, I took a very small metal ring and ran the ends through it, so when I pull the right one up and place it in my ear, the ring holds the left one from sliding down, or from around my neck.
